Second, the cause of the accident

In the operation box, there were originally three 15 amp screw insurances. After the insurance was replaced several times, the left and middle two phases were wound up and down with a fuse. The intermediate phase safety seat is fixed by two screws. There is only one one. The fixing is not fixed. In the process of moving the belt conveyor, the power cord is loose, and the screw safety base in the operation box is pulled to the left, resulting in the middle phase power supply line and The fuse is in contact with the iron plate of the operating box to energize the entire device.
3. Management of temporary workers is confusing. Temporary workers entering the factory, the labor and management department did not go through the formalities, the safety department did not file the case, but after a private introduction, the warehouse agreed to work in the salt store, did not sign the employment contract according to the regulations, did not carry out pre-job safety education, and there was no temporary Work management system.
